Health Care Extenders

Health care extenders are non-physician health care professionals who help people prevent and manage their health conditions. This group includes nurse practitioners, medical assistants, community health workers, pharmacists, social workers, and registered dieticians, among others.

The inclusion of health care extenders on a patient’s health team can help to reduce the prevalence of chronic disease and its associated risk factors.
