Asthma

People living with asthma can lead healthy, active lives.  Through the reduction of triggers such as; tobacco smoke, pests and mold, asthma can easily be manageable.  Community health workers can offer in-home self-management education to patients with asthma to assist in the management of asthma and to ultimately reduce asthma related hospitalizations.  Developing an asthma action plan and sharing it with the school nurse is another critical part of managing asthma.
