Project Summary

We provide patient navigation in all of our community outreach. This includes but is not limited to: facilitating cancer related screenings, assisting the client to find a medical home or other needed serves, education, follow-up, etc. There are 2 advanced practice nurses who serve in this capacity, in addition, we educate community based individuals to serve as a link to us when we are not on site.

Project Goal

Health Promotion and Chronic Disease education and navigation in areas that are underserved throughout Allegheny County and counties within our network. The goal of our community outreach program is to educate communities and more importantly, individuals about the risks of chronic disease and in particular cancer and how one can decreasing their risk. This includes, developing a healthy lifestyle, not smoking, seeing a doctor regularly, getting recommended screenings, etc. Navigation involves the identification and when possible the removal of barriers that stand in an individual’s way of meeting their goals, such as lack of transportation so they cannot go to the doctor. The goal is written within the original submission: to facilitate cancer screenings, assist clients in finding a medical home, etc

Project Results

Clients within the community are getting their recommended cancer screenings, making changes to their lifestyle in order to try to decrease their chronic disease risks. Again, many of contacts are made once or twice and we monitor compliance if we make a recommendation and a referral but we cannot follow up with everyone we make a connection with. There are no plans to keep growing. These numbers are very large for 2 individuals to achieve on a yearly basis. Maintaining would be great. The project is ongoing and continues to grow. We reach between 5-8,000 individuals yearly.
